ViaQuest Psychiatric & Behavioral Solutions in Holgate, OH seeks a Behavioral Health Case Manager. The role offers a full-time position with compensation at $18-$21 per hour.

You will work with a team of clinicians and professionals to support diverse clients including children, adolescents, adults, and those with developmental disabilities. Responsibilities include providing Therapeutic Behavioral Services, planning individualized supports, crisis prevention, and coordination of care.

Qualifications

• Bachelor's or Master's degree in a related field or high school diploma with significant experience.
• Related experience or training in mental/behavioral health conditions.
• Valid driver's license, car insurance, and reliable transportation.
• Willingness to travel throughout assigned service area.

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician.
• Develop service planning for individualized supports or care coordination and treatment plans.
• Provide linkage, interventions, and crisis prevention services.
• Deliver services to individuals or groups of clients.

Responsibilities may include:

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs.
• Provide service planning for individualized supports or care coordinator of healthcare, behavioral health, non-healthcare services and development of a treatment plan.
• Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.

Requirements for this position include:

• Must be at least 18 years of age.
• Bachelor's or Master's level degree in a related field or a high school diploma or three or more years of relevant field experience.
• Related experience or training with mental or behavioral health conditions.
• Valid driver's license, car insurance and reliable transportation.
• Willingness to travel throughout assigned service area.
