Job summary

ViaQuest Psychiatric & Behavioral Solutions is seeking a Behavioral Health Case Manager in Brook Park, OH. This full-time role runs Monday through Friday, 9am-5pm, with an hourly pay of $18–$21 plus a monthly incentive for productivity above target.

The team provides care to diverse client groups using a collaborative, multidisciplinary approach. A valid driver’s license and travel across the service area are required, with full benefits and PTO offered.

Qualifications

• Must be at least 18 years old.
• High school diploma plus relevant field experience or higher degree.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide Therapeutic Behavioral Services to clients with clinician support.
• Lead service planning and care coordination for behavioral health needs.
• Link with treatment options and crisis prevention services.
• Deliver services to individuals or groups and coordinate with outside contacts.
• Promote a positive, cooperative relationship with contacts.

Our clients are supported by a team of mental health counselors, social workers, certified nurse practitioners, case managers and psychiatrists to ensure the best outcomes for those we serve.

Responsibilities may include:

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs.
• Provide service planning for individualized supports or care coordinator of healthcare, behavioral health, non-healthcare services and development of a treatment plan.
• Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.

Requirements for this position include:

• Must be at least 18 years of age.
• High school diploma plus three or more years of relevant field experience or Bachelor's or Master's level degree in a related field.
• Related experience or training with mental or behavioral health conditions.
• Must have a valid driver's license and reliable transportation.
• Willingness to travel throughout assigned service area.
