Job summary

ViaQuest Psychiatric & Behavioral Solutions seeks a Behavioral Health Case Manager in Stow, OH. This full-time role runs Monday–Friday, 9am–5pm, with a pay range of $18–$21 per hour plus a monthly incentive for meeting productivity goals.

Responsibilities include delivering Therapeutic Behavioral Services, developing individualized treatment plans, coordinating care, and providing crisis prevention and life-skill restoration services to clients of various ages and needs.

Qualifications

• High school diploma plus 3+ years in mental/behavioral health.
• Bachelor's or Master's degree in a related field preferred.
• Must have a valid driver's license and reliable transportation.

Responsibilities

• Provide Therapeutic Behavioral Services by consulting with a licensed clinician.
• Develop service plans and care coordination across healthcare and behavioral health.
• Offer crisis interventions and restore daily life skills.
• Deliver services to individuals or groups as needed.
• Maintain positive relationships with outside contacts.

Behavioral Health Case Manager

Responsibilities may include:

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs.
• Provide service planning for individualized supports or care coordinator of healthcare, behavioral health, non-healthcare services and development of a treatment plan.
• Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.

Requirements for this position include:

• Must be at least 18 years of age.
• High school diploma plus three or more years of relevant field experience or Bachelor's or Master's level degree in a related field.
• Related experience or training with mental or behavioral health conditions.
• Must have a valid driver's license and reliable transportation.
• Willingness to travel throughout assigned service area.
