Job summary

ViaQuest Psychiatric & Behavioral Solutions in Holgate, OH is seeking a Behavioral Health Case Manager to join our care team. You will coordinate services, support clients across age groups, and work with licensed clinicians to develop effective treatment plans.

The role requires a bachelor’s or master’s degree in a related field or substantial experience, a valid driver's license, and willingness to travel within the service area. Full-time with strong benefits offered.

Qualifications

• Bachelor's or Master's degree in a related field, or a high school diploma with 3+ years of relevant field experience.
• Valid driver's license, car insurance, and reliable transportation.
• Willingness to travel throughout the assigned service area.
• Experience with mental or behavioral health conditions preferred.

Responsibilities

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs.
• Provide service planning for individualized supports or care coordinator of healthcare, behavioral health, non-healthcare services and development of a treatment plan.
• Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.
