Job summary

ViaQuest seeks a Behavioral Health Case Manager to join our team in Hudson, OH. This full-time role runs Monday through Friday from 9am to 5pm.

Compensation is $18-$21 per hour, based on education and experience, plus a monthly incentive program for exceeding productivity expectations. Responsibilities include providing Therapeutic Behavioral Services, planning individualized supports, coordinating care, and offering crisis prevention and life-skill restoration services to clients.

Qualifications

• Requires high school diploma and 3+ years of relevant field experience, or a Bachelor's/Master's degree in a related field.
• Must have a valid driver's license and reliable transportation.
• Willingness to travel within the assigned service area.

Responsibilities

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs.
• Provide service planning for individualized supports or care coordinator of healthcare, behavioral health, non-healthcare services and development of a treatment plan.
• Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.
