Job summary

Community Partners NH in Strafford County seeks a Resource Center Intake Specialist to support the I&R team. You will link clients with community service providers and assist with case management for behavioral health and family clients.

Responsibilities

• Provide intake support as part of the I&R team, linking clients to community service providers.

• Assist in coordinating services to help clients live independently within the community.

The Role

The Resource Center Intake Specialist provides support to the agency and serves as part of the Information and Referral (I&R) team. You will be linking clients with appropriate community service providers, and organizations that empower individuals to live independently within their community. You will also be providing case management support for behavioral health, adult and youth and family clients.

When someone asks you for directions, you not only tell them the way, but you also help enter their desired location into a GPS, draw a detailed map on a napkin, then send them on their way with encouragement and a smile. What We’re Looking For
