Job summary

Community Partners in New Hampshire seeks a Case Manager to organize, facilitate, and document service plans for individuals with Developmental Disabilities. You will collaborate with community resources to ensure quality service provision and client outcomes.

Requirements include an associate degree (Bachelor’s preferred), 2–3 years of relevant experience, strong communication skills, and a valid driver’s license.

Qualifications

• Associate degree or equivalent required, Bachelor's Degree preferred
• Two-three years of relevant field experience preferred
• Excellent oral and written communication skills
• Ability to utilize technology provided to perform requirements of the job
• Hold a valid driver’s license and proof of vehicle liability coverage
• Must demonstrate excellent multitasking abilities

The Role: As the Case Manager, you will be responsible for organizing, facilitating, and documenting the service planning for individuals with Developmental Disabilities as well as working collaboratively with other community resources to ensure the quality of service provision.
