Job summary

ViaQuest Psychiatric & Behavioral Solutions is seeking a Behavioral Health Case Manager to join our team in Bath, OH. This full-time role runs Monday–Friday, 9am–5pm, with a pay range of $18–$21 per hour based on education and experience, plus a monthly incentive program for productivity.

The position supports children, adolescents, adults, the aging population, and those with developmental disabilities or trauma.

Qualifications

• Must have a valid driver’s license.
• Willingness to travel within assigned service area.
• Experience in behavioral health or related field preferred.

Responsibilities

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs.
• Provide service planning for individualized supports or care coordination of healthcare, behavioral health, non-healthcare services and development of a treatment plan.
• Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.

Responsibilities may include:

Requirements for this position include:

• Must be at least 18 years of age.
• High school diploma plus three or more years of relevant field experience or Bachelor's or Master's level degree in a related field.
• Related experience or training with mental or behavioral health conditions.
• Must have a valid driver's license and reliable transportation.
• Willingness to travel throughout assigned service area.
