Job summary

ViaQuest Behavioral Health is seeking a part-time Behavioral Health Case Manager in Brunswick, Ohio. The role involves providing therapeutic behavioral services, care planning, and linkage to resources to support clients across age groups and settings.

Responsibilities include crisis prevention, assisting with treatment plans, and coordinating care with licensed clinicians. A valid license and ability to travel within the service area are required.

Qualifications

• Bachelor's or Master's degree in a related field, or a high school diploma with 3+ years of relevant experience.
• Related experience or training with mental or behavioral health conditions.
• Valid driver's license, car insurance and reliable transportation.
• Willingness to travel throughout assigned service area.

• Provide Therapeutic Behavioral Services to clients by consulting with a licensed clinician to assist with the individual's needs.
• Provide service planning for individualized supports or care coordination of healthcare, behavioral health, non-healthcare services and development of a treatment plan.
• Provide linkage, interventions, treatment options, restoration of daily life skills and crisis prevention services.
• Provide services to individual clients or groups of clients.
• Promote a positive and cooperative relationship with all outside contacts.

Education

Bachelor's or Master's degree in related field

High school diploma with 3+ years of relevant experience

Experience or training in mental/behavioral health

Valid driver's license and reliable transportation

Willingness to travel throughout assigned service area
