Qualifications

Responsibilities

• Discharge planning including coordination with Social Services for aftercare arrangements and transportation.
• Assist with completion of patient satisfaction surveys and clinical outcomes surveys.
• Initiate follow-up with external referral sources and manage care point alerts.
• Audit discharge data and paperwork to ensure accuracy before transition home or to a community-based program.
• Ensure discharge transition records are sent to receiving providers.
• Perform other duties as assigned.
