Job summary

Adjuvant Behavioral Health is seeking a Care Coordination Specialist in Weatherford, TX. This in-person position serves as the first contact for patients referred to behavioral health services and coordinates consent, handoffs, and service delivery within the CoCM model.

You will document consents, maintain EHR records, and collaborate with BHCMs and providers to ensure timely referrals and continuity of care, while upholding HIPAA and ethical standards.

Qualifications

• Bachelor’s degree required with healthcare exposure preferred.
• Strong interpersonal and communication skills to engage patients.
• Detail-oriented with solid documentation practices.

Responsibilities

• Serve as the initial liaison and point of contact for patients.
• Conduct informed consent discussions and document consents.
• Coordinate referrals to Behavioral Health Care Managers (BHCMs).
• Maintain accurate EHR documentation and HIPAA compliance.
• Support care team with coordinating behavioral and psychosocial services.
• Monitor referrals, follow up with patients, and participate in team meetings.
