Job description

Position Overview

The Care Coordination Specialist plays a key role in the Collaborative Care Management (CoCM) model within our behavioral health practice. This role serves as the first point of contact for patients referred into behavioral health services. The Specialist is responsible for obtaining patient consent for participation in Collaborative Care, ensuring seamless handoff of referrals to Behavioral Health Care Managers, and supporting the care team by coordinating services that address patients’ behavioral and psychosocial needs.


Key Responsibilities


  • Serve as the initial liaison and point of contact for patients referred to behavioral health services.

  • Conduct informed consent discussions with patients, ensuring understanding of the Collaborative Care Model.

  • Collect and document patient consents in compliance with organizational and regulatory standards.

  • Facilitate warm handoffs and timely referrals to Behavioral Health Care Managers (BHCMs).

  • Maintain accurate and thorough documentation in the electronic health record (EHR).

  • Coordinate with medical providers, BHCMs, and psychiatric consultants to ensure continuity of care.

  • Monitor referral completion and follow up with patients as needed.

  • Provide patients with information, resources, and support to improve access to care.

  • Participate in care team meetings and communicate relevant updates to the behavioral health team.

  • Maintain compliance with HIPAA and ethical standards of professional practice.


Qualifications


  • Bachelor's Degree

  • Strong interpersonal and communication skills with the ability to engage patients sensitively, clearly and effectively.

  • Knowledge of behavioral health and medical systems of care.

  • Detail-oriented with strong organizational and documentation skills.

  • Ability to work collaboratively in a multidisciplinary team environment.

  • Prior experience in healthcare sales, business development, outward facing patient contact, preferably in an outpatient setting.

  • Organized, detail-oriented; proficient in Microsoft Office tools (Excel, Word, Outlook, PowerPoint or comparable).

Preferred Experience


  • Prior experience in care coordination, patient navigation, or case management.

  • Familiarity with the Collaborative Care Model (CoCM) and/or integrated behavioral health.

  • Experience in oncology, chronic illness, or primary care settings.

  • Background in business development, hospitality, or service industries in client-facing roles. Brings an enthusiastic, customer-first approach with an ability to attract, engage, and retain clients/patients in a growth-oriented setting.

  • Bilingual (Spanish/English) strongly preferred.


Work Environment & Schedule


  • Full-time, in-person role based in the Weatherford, TX area.

  • This role begins as a 1099 contractor position, with the option to reevaluate and transition to W‑2 employment status after 90 days.

  • Periodic travel within Weatherford, TX area.

  • Requires flexibility to support patient schedules and occasional team meetings.
