Job summary

Molina Healthcare seeks a Florida-licensed behavioral health professional (LCSW, LMHC, or LMFT) with Medicaid experience to lead care coordination and transitions across inpatient, outpatient, and community settings.

The role requires strong collaboration with multidisciplinary teams, crisis intervention, discharge planning, and connecting members to behavioral health resources to improve outcomes and reduce readmissions; approx 10% local travel is expected.

Qualifications

• 2+ years in health care, care management, or behavioral health setting.
• Licensed clinician required per state contract or regulation.
• Valid driver's license and reliable transportation for local travel.
• Knowledge of community resources and care coordination.
• Ability to work independently and handle multiple priorities.

Responsibilities

• Complete member assessments within regulatory timelines.
• Develop and implement care plans with stakeholders.
• Conduct telephonic, in-person, or home visits.
• Monitor care plan progress and adjust interventions.
• Maintain an ongoing member caseload and outreach.
• Promote integration of behavioral health and community resources.
• Facilitate ICT meetings and collaboration.
• Utilize motivational interviewing techniques.
• Identify and address barriers to care.
