Centene is seeking a Hybrid Behavioral Health Care Manager to evaluate and coordinate care for members with significant mental/behavioral health needs in Texas. You will conduct home and field visits, develop care plans, educate families, and collaborate with providers to ensure access to services.

Requires an unrestricted behavioral health license and a master’s degree in Mental Health Counseling or Social Work, with 2–4 years of related experience; travel is required.

Formación

• Requires a master’s degree in Mental Health Counseling or Social Work.
• Unrestricted behavioral health license as LCSW/LMFT/LMHC/LPC for the applicable state.
• 2–4 years of related experience in behavioral health or care management.

Responsabilidades

• Evaluate needs of high-risk members with mental/behavioral health needs and develop care plans.
• Act as liaison between member/family, physician, and facilities.
• Conduct frequent home or site visits to assess needs and coordinate resources.
• Educate members and families on preventive care, treatment options and benefits.
• Ensure documentation and adherence to clinical guidelines and state/federal requirements.
