Job summary

The Behavioral Health Care Manager at UnityPoint Health integrates the care of the individual within Behavioral Health Services. The role works in a multidisciplinary team, offering elements of therapeutic modalities and serving as a liaison for the patient and other services to ensure seamless care.

Responsibilities include utilization management, discharge planning, and coordination of a multidisciplinary plan of care across the health care continuum.

Qualifications

• Bachelor of Arts/Science Degree in a health care/human services related field or RN.
• Master’s degree in a healthcare/human services related field is highly encouraged.
• Two years of clinical experience in focused areas working with multidisciplinary teams.
• Licensure appropriate to population served.
• Valid driver’s license when driving any vehicle for work-related reasons.

Responsibilities

• Addresses and monitors length of stay issues and level of care changes for compliance.
• Documents the case management plan to include clinical needs, barriers to quality care, and resource utilization; pursues referrals promptly.
• Collects data, analyzes trends, and reports on patterns of care and resource utilization.
• Assess the individual's health status and coordinate referrals to multi-disciplinary services.
• Monitors and coordinates an interdisciplinary plan of care across the health care continuum and during transitions.
• Develops and implements an effective discharge plan for complex-care patients and coordinates transitions.
• Demonstrates knowledge of financial and reimbursement processes to support medical cost management.
• Educates patient, family, staff and physicians regarding benefits, utilization of resources, levels of care, and transition expectations.
