Job summary

Daymark Recovery Services in Mocksville, NC seeks a qualified Care Manager to perform assessments, develop person-centered care plans, coordinate referrals, and monitor progress for individuals with behavioral health needs.

Under supervision, you will collect medical/psychosocial information, collaborate with providers, ensure thorough documentation, and support clients toward outcomes and access to services.

Qualifications

• An Associates or bachelor's degree in a human service field with two years MH/SA/DD experience.
• OR a licensed RN with two years MH/SA/DD experience.
• OR a Masters with licensure in a human service field with one year MH/SA/DD experience.
• OR Bachelor’s outside of human service field with four years MH/SA/DD experience.

Responsibilities

• Documents client service needs, strengths, resources, preferences, and goals to develop a Care Management Plan.
• Performs care management assessments and revises plans as needed.
• Coordinates referrals to medical, behavioral health, and social services.
• Monitors progress and documents outcomes, updating the care plan accordingly.
• Collaborates with providers to ensure continuity of care and access to services.
• Maintains timely documentation and client records for audits and quality metrics.
