Job summary

Advantage Healthcare Services in Hawaii seeks a Community Health Liaison to support behavioral health patients in clinic, parole, and home settings while coordinating care with clinical and pharmacy teams to improve medication adherence and health outcomes. Work involves Enhanced Clinical Services, MTM, and direct patient visits.

You will collaborate with care teams, assist with insurance processes, and engage with partner organizations to enhance treatment effectiveness.

Qualifications

• High school diploma or equivalent required.
• Completion of a Community Health Worker training program preferred.
• Experience in community-based settings and health terminology helpful.
• Strong communication, empathy, and motivational skills.

Responsibilities

• Conduct Enhanced Clinical Services for AHCS patients.
• Provide MTM and other clinical services to optimize care.
• Visit patients in clinics, parole, and at home-based settings.
• Coordinate care with healthcare providers and care teams.
• Support insurance coverage and enrollment efforts as needed.
• Collaborate with AHCS sales on referrals and outreach.
• Maintain timely reporting and documentation.
