Job summary

HMSA Careers is seeking a community health worker to build trusting relationships with individuals and families, coordinate with clinicians, and promote health, recovery, and wellness in the Hawaii community.

The role involves motivational interviewing, education, referrals, and navigating healthcare systems to ensure timely, appropriate services and improved member outcomes. Hybrid work arrangement is indicated.

Qualifications

• Requires excellent communication skills (written and verbal).
• Associate's degree in healthcare/social services or equivalent experience.
• Fieldwork including home visits, health screenings, navigation of social services.

Responsibilities

• Manage community health by establishing trusting relationships with individuals, families, and providers to promote health, recovery, resiliency, and wellness.
• Coordinate with licensed clinicians and Health Management programs to ensure optimal care.
• Use motivational interviewing to encourage provider appointments and close care gaps.
• Deliver education, resources, referrals, and connections to health services within the community.
• Guide community members through complex cases and navigate healthcare systems, including home visits as needed.
• Provide culturally appropriate health education and information on using health services.
• Perform other duties as assigned.
