Job summary

Advantage Healthcare Services is seeking a Community Health Liaison to support Enhanced Clinical and Support Service Programs, working with behavioral health patients in clinic, parole, and home-based settings. You will coordinate care, provide patient support, and help improve medication adherence and health outcomes.

The role involves field visits, cross-functional collaboration with clinical and pharmacy teams, and reporting requirements.

Responsibilities

• Collaborate with AHCS sales on potential referrals.

Job Overview:

In the Community Health Liaison (CHL) role, you will work under the supervision of the Community Health Liaison Manager. You willsupport AHCS Enhanced Clinical and Support Service Programs by working directly with behavioral health patients in clinic, parole, and home-based settings to improve medication adherence and overall health outcomes. You will alsoassistincoordinating care with clinical and pharmacy teams, providing patient support services, and identifying gaps in care to enhance treatment effectiveness. Success in this role will bedemonstratedbyimproved patient engagement, adherence to treatment plans, and measurable optimization of health outcomes.

• Attend meetingsand workwith other clinical team members
• Complete required reporting requirementsin a timely manner
• Act as liaison between patient and pharmacy to ensuremedication adherence
• Assistpharmacy teams with insurance coverage lapses and benefit enrollment to ensure continuity of pharmacy care
• Collaborate with AHCS sales team on potential new referral source, leads, and customers
• Perform other tasks as required

Required Skil ls:

• High school diploma or equivalent
• Ability to work in a multi-cultural setting
• Proficiency in Microsoft Office
• Understandinging ofthe behavioral health community
• Effective communicationskillsverbally and in writing
• Ability and willingness to provide emotional support,encouragement,and motivation to patients

Education and Experience:

• Experience working in a community-based setting for 1 to 2 years
• Previous experience in Medical Assistant, Community Health Worker, Social Worker, Pharmacy Technician, Pharmacy Clerk, or equivalent
• Successful completion of a Community Health Worker training program
• Medical terminology and/or healthcare background

Working Conditions:

• Full-time, typically requiring 40+ hours per week
• Field work will require driving
• Work is performed in a healthcare setting, including hospitals, clinics, and outpatient facilities, or visiting patient homes
• Ability to work flexible hours, including weekends and evenings as needed
