Job summary

Qualifications

• Bachelor's degree in health or human services field or LPN.
• 1+ year of experience working directly with individuals with severe mental illness.
• 1+ year of experience with MS Office (Word, Excel, Outlook).
• Driver's license and reliable transportation and ability to travel within territory.

Responsibilities

• Be the primary care manager for a panel of serious mental illness (SMI) members.
• Complete Health Risk Assessments and Individualized Service Plans.
• Link members to needed services to ensure appropriate healthcare utilization.
• Collaborate with physicians, behavioral health providers and community partners; conduct face-to-face visits 25–50% of the time.
