Job summary

Apalachee Center, Inc. in Tallahassee, FL, seeks a Care Coordinator to engage priority individuals and coordinate services across CSU, detox, housing, and primary care, ensuring warm handoffs and continuity of care.

The role requires a bachelor's degree in a related field and experience with adults experiencing serious mental illness; LOCUS, Avatar documentation, and cross-system collaboration are core components of daily work.

Qualifications

• Bachelor's degree in counseling, social work, psychology, criminal justice, nursing, rehabilitation, health education, health science, or related field.
• Three years full-time or equivalent experience working with adults experiencing serious mental illness preferred.

Responsibilities

• Utilize LOCUS level of care tool to identify service needs for the individual served.
• Serve as the single point of accountability for coordinating care with all involved parties.
• Engage the individual in their current setting (CSU, Detox, homeless shelter, home, etc.).
• Develop a client service plan based on shared decision making emphasizing self-management, recovery and wellness.
• Meet Service Productivity standards for care coordination and ensure proper documentation per guidelines.
• Document all services in the client record; maintain Avatar notes within 48 hours; ensure high compliance with progress notes.

Education

Bachelor's degree in counseling, social work, psychology, criminal justice, nursing, rehabilitation, health education, health science, or related human services field

Three years full-time or equivalent experience working with adults experiencing serious mental illness (preferred)
