Job summary

Qualifications

• Bachelor's degree in counseling, social work, psychology, criminal justice, nursing, rehabilitation, or related human services field.
• One year full-time or equivalent experience working with adults with serious mental illness preferred.
• Alternate bachelor’s degree with three years full-time or equivalent experience working with adults with serious mental illness preferred.

Responsibilities

• Utilize LOCUS tool and assessments to identify service needs.
• Coordinate care as the single point of accountability across involved parties.
• Engage the individual in various settings (CSU, Detox, shelter, home, etc.).
• Develop a client service plan based on shared decision making emphasizing self-management and recovery.
• Meet Service Productivity standard of at least 75% for care coordination.
• Document progress notes in Avatar within 48 hours; ensure compliance with guidelines.
• Provide frequent contact for the first 30 days (daily to at least three times per week).
