Job summary

Apalachee Center is seeking a Family Care Coordination Peer Specialist in Tallahassee to engage youth and their families, provide mentorship, and guide them through the care system. The role focuses on recovery, engagement, and linking families to community resources to improve outcomes.

The ideal candidate brings lived experience with mental health or substance use recovery, is certified or eligible for certification as a Peer Specialist, and can collaborate with a multidisciplinary team to

Qualifications

• High school diploma or its equivalent.
• Lived experience with recovery from a substance use or mental health condition as an adult OR as a family member/caregiver.
• Certified as a Peer Specialist or eligible to obtain certification.
• Experience with mental illnesses and/or substance use disorder.

Responsibilities

• Use lived experience to assess needs and provide peer support and guidance.
• Engage the individual and family in discharge planning and connect to community services.
• Collaborate to build self-directed recovery tools like WRAP.
• Provide frequent contact for first 30 days to assess safety and independence.
• Document activities and maintain client records.
• Promote a team culture that reduces stigma and values each perspective.
• Engage youth and families to set recovery goals and teach coping and problem-solving skills.
