Qualifications

• Bachelor's Degree in Social Work, Psychology, or a related field.
• At least 1 year of experience with diverse populations including African American, Indigenous, and LGBTQIA+ communities.
• 1 year experience presenting information to individuals and/or groups; comfortable with public speaking.
• Excellent verbal, written, and interpersonal communication in a multi-cultural environment.
• Proficient in MS Word, MS Excel and computer tracking database.
• Valid Minnesota driver's license with good driving record and current insurance.
• Bi-lingual in Hmong, Somali or Spanish is a plus.
• Flexibility to work some evening and weekend hours.

Responsabilités

• Manage referrals to the Behavioral Health Department from internal and external partners and self-referrals.
• Follow up on each referral and complete intake for behavioral health services.
• Complete registration and insurance activities, schedule appointments, and document all services.
• Provide outreach to adults and families in North Minneapolis and surrounding communities.
