Job summary

NorthPoint Health & Wellness Center in Minneapolis seeks a Behavioral Health Intake and Outreach Coordinator to manage referrals, conduct intake, and coordinate registrations and scheduling.

You will conduct outreach to adults and families in North Minneapolis, document services, and work with diverse communities. A Bachelor's degree and one year of experience are required; bilingual language skills are a plus.

Qualifications

• Bachelor's degree in social work, psychology, or related field required.
• Minimum 1 year experience working with diverse populations.
• Excellent verbal, written, and interpersonal communication skills.
• Valid Minnesota driver's license with reliable vehicle and insurance.
• Bi-lingual in Hmong, Somali or Spanish is a plus.
• Flexibility to work some evenings and weekends.

Responsibilities

• Manage referrals to the Behavioral Health Department from internal and external partners.
• Follow up on referrals and complete intake for behavioral health services.
• Complete registration and insurance activities and schedule appointments.
• Document all services provided.
• Provide outreach to adults and families in North Minneapolis and surrounding communities.

General Job Statement:

The Behavioral Health Intake and Outreach Coordinator is responsible for managing referrals made to the Behavioral Health Department by both internal and external partners as well as patients who may self-refer. The staff will be responsible for following up on each referral received, completing an intake for behavioral health services, completing registration and insurance activities, scheduling an appointment, and documenting all services provided. The staff member will also provide outreach to adults and families residing in North Minneapolis and surrounding communities who may benefit from behavioral health services.

Minimum Qualifications:

• Bachelor's Degree in Social Work, Psychology, or other similar field, required.
• Minimum experience of 1-year working with diverse populations including, but not limited to, African American, Indigenous, and LGBTQIA+ communities.
• 1-year experience presenting information to individuals and/or groups. Must be comfortable and competent with public speaking.
• Excellent verbal, written, and interpersonal communication skills working with a multi-cultural environment.
• Proficient in MS Word, MS Excel and computer tracking database.
• Valid Minnesota driver's license with good driving record, reliable vehicle with current insurance and ability to meet NorthPoint's insurance criteria.
• Bi-lingual in Hmong, Somali or Spanish, a plus.
• Flexibility to work some evening and weekend hours
