Stormont Vail HealthCare seeks a Behavioral Health Admissions & Nurse Navigator to coordinate patient access and placement across the behavioral health continuum. You will conduct psychiatric assessments and collaborate with clinicians to ensure timely admission and smooth patient movement.

The role emphasizes strong communication, crisis management, and navigating multiple systems to improve access and patient experience while complying with regulatory standards.

Qualificações

• Master's degree in a related field required or licensed RN substitution.
• 1 year psychiatric or acute care experience required.
• Experience in intake, admissions, care coordination, or navigation preferred.
• Ability to work with multidisciplinary teams and manage regulatory requirements.

Responsabilidades

• Lead and streamline the behavioral health admission process with a one-call model.
• Conduct comprehensive psychiatric assessments across age groups to determine level of care.
• Facilitate admissions to inpatient and outpatient services based on criteria.
• Coordinate alternative placements when needed and address admission barriers.
• Ensure intake documentation is complete and regulatory compliant.
• Guide patients and families from intake through episode of care.
• Coordinate with psychiatrists, nurses, social workers, case managers and community providers.
• Verify insurance benefits and coordinate prior authorizations as needed.
• Document interactions in the electronic health record and maintain records.
