Job summary

MN Care Services is seeking compassionate direct support professionals for part-time morning, afternoon, and overnight rotations in Minnesota. The role involves implementing care plans, data collection, and supporting daily living activities with a focus on person-centered care.

You will document thoroughly, collaborate with care teams, and assist individuals in social integration and community activities while maintaining HIPAA compliance.

Qualifications

• High school diploma or GED with 3+ years in care settings.
• Must be 21+, valid driver's license with clean record.
• Ability to pass MN DHS background check with fingerprinting.
• Must complete required 245D training competency.
• Preferred: 245D/HCBS experience, BSP/BIPs, data documentation, person-centered approaches.
• Strong de-escalation and behavioral understanding.

Responsibilities

• Follow care plans and document shift notes and medication administration reports.
• Collect accurate data to support the person’s care plan.
• Assist with meal planning, shopping, housekeeping, and self-care tasks.
• Provide skill training to empower individuals served.
• Support decision-making and choices in daily living and service providers.
• Transport individuals to destinations using company vehicle.
• Coach individuals during escalations and crises.
• Maintain positive relationships with staff, coordinators, and families.
• Adhere to Vulnerable Adult/Child policies, HIPAA, and confidentiality.
• Perform cross-functional duties as needed.
