Job summary

MN Care Services is hiring a caregiver/support professional to follow individualized care plans, document activities, and collect data to drive positive outcomes. The role includes assisting with daily living tasks, problem-solving, and empowering individuals through skill-building.

You will support decision-making, community participation, and remote or on-site duties while maintaining HIPAA and confidentiality standards. Competitive hourly pay offered.

Qualifications

• High school diploma or GED plus 3+ years of progressive experience in behavioral, residential, or mental health settings.

Responsibilities

• Follow the person’s care plan and complete all relevant documentation on time and accurately.
• Collect accurate data to drive positive behaviors per the care plan.
• Provide problem-solving support for meal planning, shopping, housekeeping, and self-care.
• Advocate for and empower individuals with general skill training.
• Support decisions about daily living, goals, and everyday choices.
• Transport individuals to their destinations using company vehicles when needed.
