Job summary

MN Care Services in Minnesota is seeking part-time Direct Support Professionals to work with individuals in residential settings, following individualized care plans, and documenting interactions and outcomes accurately.

You will help build daily living skills, support community participation, assist with meal preparation, budgeting, housekeeping, and ensure safe, respectful environments while maintaining open communication with the care team.

Qualifications

• High school diploma or GED with 3+ years in behavioral, residential, or mental health settings.
• Age 21+; valid driver’s license with clean record.
• Ability to pass MN DHS background study with fingerprinting.
• Ability to complete and demonstrate competency in required 245D training.
• Experience in 245D/HCBS settings; BSPs/BIPs; structured data collection; person-centered and trauma-informed approaches.

Responsibilities

• Follow the person’s care plan and ensure documentation (shift notes, medication administration reports) is completed timely and accurately.
• Collect accurate data to drive positive behaviors as part of the care plan.
• Provide problem-solving assistance with activities such as meal planning, grocery shopping, housekeeping, cooking, organizational skills, budgeting, and self-care.
• Advocate for and empower individuals served by providing general skill training.
• Support decision-making and daily living goals, including choice of service providers, goals, location, home decor, leisure activities, roommates, and personal possessions.
• Transport individuals to destinations via company vehicle; facilitate community involvement.
• Coach individuals through escalations in behavior or mood and during emergencies.
• Maintain positive relationships with teammates, Designated Coordinator, and care teams; ensure confidentiality and HIPAA compliance.
• Perform cross-functional duties as needed.
