Qualifications

• ACLS required within one year of hire.
• Bachelor's Degree in Nursing or equivalent is preferred; 2+ years of relevant experience may be accepted in lieu of a degree.
• Strong communication and interpersonal skills with ability to collaborate across multidisciplinary teams.

Responsibilities

• Assess, plan, develop, implement and evaluate plans of care for defined patient populations.
• Coordinate with multidisciplinary team to facilitate appropriate and timely patient placement and transfers.
• Communicate capacity management plans to nursing units, leadership, and physicians.
• Serve as a clinical resource when department leadership is unavailable.
