Job summary

WelbeHealth, LLC. seeks an accomplished AVP, Home Care to lead strategy, operations, quality, and compliance across multiple markets.

You will partner with Clinical, Operational, Finance, and People teams to ensure high-quality participant care, scalable operations, and regulatory readiness. The role demands hands-on leadership across Home Care operations, staffing optimization, and performance improvement to support aging-in-place for vulnerable seniors within a growing national PACE

Qualifications

• Bachelor’s degree required; master’s preferred.
• Clinical licensure or equivalent healthcare leadership experience preferred.
• 7–10 years of progressive leadership in health services administration, home care, or related programs; multi-site leadership preferred.
• Minimum of 5 years of people leadership with distributed operations experience.
• Strong operational and financial acumen, including budgeting, workforce planning, productivity, and performance improvement.
• Deep knowledge of home care operations, regulatory compliance, quality management systems, and audit readiness.

Responsibilities

• Provide executive leadership for the home care function across multiple sites, setting direction and ensuring high-quality service delivery.
• Lead regional and market-level Home Care leaders, coaching and developing teams.
• Oversee multi-site operations, staffing models, scheduling, vendor management, and capacity.
• Establish KPIs, dashboards, and operating rhythms to hit quality, service, utilization, compliance, and financial goals.
• Oversee care plans with providers and coordinators to ensure aging in place and plan-of-care alignment.
• Provide oversight of the home care quality management system, audits, action plans, and continuous improvement.
• Ensure compliance with federal, state, and local laws, PACE requirements, and regulatory audits.
