HealWell, LLC is seeking a motivated and compassionate APRN/PA to join our wound care team. This role provides specialized wound care to patients across private residences, skilled nursing facilities, long-term care facilities, and outpatient clinic settings.

Our providers work independently while collaborating with patients, families, facility staff, and other members of the healthcare team to develop individualized treatment plans focused on wound healing, limb preservation, and improving

Qualificações

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current, unrestricted APRN/PA license with prescriptive authority in the State of Oklahoma.
• Current national board certification.
• Wound care certification preferred; may be required within six months of hire.

Responsabilidades

• Perform comprehensive patient and wound assessments.
• Develop and manage individualized wound care treatment plans.
• Accurately assess, measure, stage, and document wounds and wound characteristics.
• Perform wound care procedures within scope of practice, including debridement as appropriate.
• Utilize advanced wound care products, therapies, and treatment modalities.
• Assess vascular status and perform vascular screening when indicated.
• Coordinate referrals, diagnostic testing, and follow-up care.
• Collaborate with physicians, nursing staff, home health agencies, facility staff, patients, families, and others.
• Educate patients and caregivers on wound care, prevention, nutrition, and offloading.
• Maintain accurate clinical documentation.
• Adhere to HIPAA, state/federal regulations and HealWell policies.
• Maintain equipment and supplies; travel to patient locations as required.
