Job summary

Wound Care Specialists is seeking a skilled Advanced Practice Provider (NP or PA) to provide mobile wound care in patients' homes, assisted living facilities, and other community settings in a part-time/PRN capacity. This autonomous clinician will collaborate with physicians and home health teams to deliver evidence‑based wound care that aims to improve outcomes and preserve limbs.

Qualifications

• Current NP or PA license in your state of practice.
• Current NP or PA Board Certification.
• Minimum of 2 years of clinical experience preferred.

Responsibilities

• Perform comprehensive wound evaluations in home-based settings.
• Diagnose and manage acute, chronic, and complex wounds.
• Perform bedside procedures (debridement, incision and drainage, suturing where credentialed).
• Develop and implement individualized wound treatment plans.
• Coordinate care with home health nurses, primary care providers, and specialists.
• Document thoroughly in the EMR per Medicare and payer guidelines.
