Job summary

Woundtech in Moreno Valley, CA is seeking a Nurse Practitioner or Physician Assistant to join our wound care team. You will perform home visits to manage wound care and document visits promptly.

The role requires NP/PA certification, state license, autonomy, and 1–3 years of advanced practice experience. You will debride wounds, apply treatments, coordinate with PCP, and educate patients.

Qualifications

• Must hold Nurse Practitioner or Physician Assistant license in the state.
• NP/PA graduates with accredited programs; professional certification preferred.
• Relevant experience in advanced practice (1–3 years).

Responsibilities

• Provide hands-on care during patient visits.
• Debride wounds using sharp, enzymatic, autolytic, or surgical methods.
• Apply topical wound care and related products.
• Recommend additional services to improve wound healing outcomes.
• Interpret diagnostic tests and plan appropriate follow-up.
• Prescribe medications and/or DME within scope and as per collaborative agreement.
• Assess diabetes risks per ADA guidelines.
• Educate patients and coordinate with PCP/CM/Physician.
