Home or Away is seeking a Georgia-licensed RN for a flexible, field-based Supervisory role. You will oversee in-home client care, perform initial assessments within 72 hours of referral, and guide caregiving staff to ensure high-quality outcomes.

This PRN position offers autonomy and a self-managed schedule while serving families in the community. You will conduct biweekly field visits, document care plans, and collaborate with families and the clinical team to adapt plans as needs evolve.

Qualifikationen

• Georgia RN license active and in good standing.
• Strong patient assessment, medical terminology, and safe med administration.
• Experience in long-term care, geriatrics, or in-home care preferred.

Aufgaben

• Onboard and assess new clients within 72 hours and tailor care plans.
• Conduct field supervision with biweekly in-home visits to monitor care and safety.
• Manage your own schedule by coordinating assessments and visits directly with clients/families.
• Maintain accurate clinical documentation in digital systems.
• Collaborate with families and the clinical team to update care plans as health changes.
• Liaise with community partners and families to communicate client status and care updates.
• Ensure HIPAA compliance and support internal record audits.
• Record mileage, visits, and hours for payroll processing.
