Qualifications

• Active LPN or RN license in the State of Georgia.
• Current BLS certification.
• Valid driver’s license with reliable transportation.
• Current TB screening.
• Home care experience preferred.
• Supervisory visit experience preferred.
• Familiarity with Medicaid and state home care regulations preferred.

Responsibilities

• Conduct required supervisory visits in clients’ homes per state Medicaid and agency rules.
• Observe and evaluate caregiver performance during visits.
• Assess client condition and note changes impacting care.
• Ensure services align with the approved plan of care.
• Document findings accurately in the agency system and report to leadership.
