Qualifications

• Strong written and verbal communication in stressful family settings.
• Ability to manage an independent caseload and maintain documentation.
• Collaborative approach with physicians, providers, and community partners.
• Valid driver's license and reliable transportation for field visits.
• Comfort with field-based travel and non-office work environment.

Responsibilities

• Conduct comprehensive in-home assessments covering functional, cognitive, psychosocial, and environmental needs.
• Develop and maintain individualized care plans, revising them as circumstances change.
• Coordinate physicians, home care, specialists, and community resources into a single coherent plan.
• Support clients and families through transitions of care, closing gaps where people are lost.
• Provide emotional support and crisis intervention for families in need.
• Advocate for clients at appointments, care conferences, and decision points.
• Serve as primary contact for caseload clients, including urgent needs outside standard hours.
• Field-based role with high-touch nursing support and accompanying clients to appointments.
