Unicity Care Management is seeking a Social Work Care Manager and Patient Advocate in New Jersey. You will conduct in-home assessments, develop care plans, and coordinate care across physicians, home care, and community resources.

This field-based role requires travel and offers a hybrid schedule, with a focus on client advocacy, emotional support, and transitions of care. Excellent communication and organizational skills are essential.

Qualifications

• Active Licensed Social Worker (LSW or LCSW) in New Jersey.
• Interest in working alongside families, not only clients.
• Sound professional judgment in community settings without institutional resources.
• Strong written and verbal communication for families under stress.
• Organizational discipline to manage an independent caseload and documentation.
• Collaborative approach with physicians, providers, and community partners.
• Valid driver’s license and reliable transportation for field visits.

Responsibilities

• Conduct comprehensive in-home assessments covering functional, cognitive, psychosocial, and environmental needs.
• Develop and maintain individualized care plans, revising them as circumstances change.
• Coordinate physicians, home care, specialists, and community resources into a single plan.
• Support clients and families through transitions of care, closing gaps where needed.
• Provide emotional support and crisis intervention when families need it most.
• Advocate for clients at appointments, care conferences, and decisions.
• Serve as primary contact for caseload clients, including urgent needs outside business hours.
• Travel frequently; field-based role offering high-touch nursing support during visits.
