Job summary

Corewell Health West Visiting Nurses is seeking an experienced Registered Nurse to provide autonomous home health nursing across Byron Center, Dorr, and surrounding areas. You will implement treatment plans, monitor patients, and collaborate with an interdisciplinary team to ensure high-quality care in patients' homes.

Flexible scheduling includes every 4th weekend and 3rd holidays. Requires an Associate's Degree in Nursing, Michigan RN license, and BLS; incumbents may have opportunity for a

Qualifications

Associate's Degree in Nursing required.
RN license (STATE_MI) upon hire.
Driver's license (STATE_MI) upon hire.
ACLS/BLS certification (AHA ARC) required per program.

Responsibilities

Communicates effectively with staff, residents and families to assess and update individual plans of care.
Performs nursing services, including teaching, treatments, diagnostics, and preventive procedures, and evaluates patient progress.
Administers medications per regulatory guidelines and informs MDS coordinator of significant changes.
Collaborates with interdisciplinary team to ensure continuity of care and patient safety.
