Job summary

Corewell Health West Visiting Nurses Association seeks a licensed RN for home care across the West-Central Michigan area, serving Whitehall, Montague, Holton, and surrounding communities. This full-time role offers autonomy, flexible scheduling, and visits to 5–7 patients per shift in patients' homes.

Responsibilities include patient assessments, medication administration, and collaboration with the interdisciplinary team to ensure high-quality care; eligible for a sign-on bonus up to $10,000.

Qualifications

• Must have an Associate's Degree in Nursing and valid MI RN license.
• BLS certification required (AHA) for Home Care team.
• Driver's license required for travel to patient homes.

Responsibilities

• Communicates effectively with staff, residents and families; develops patient care plans.
• Administers medications per regulatory guidelines and informs MDS coordinator of changes.
• Collaborates with health care team to ensure continuity of care and coordinate services.
• Provides teaching and referrals as needed to patients and families.
• Monitors patient progress and initiates life-saving measures when needed in emergencies.
• Leads and delegates tasks to support staff to ensure safe, quality care.
• Participates in QA and process improvement activities to enhance care quality and efficiency.

Associate's Degree in Nursing

State of Michigan RN License

Driver's License

Basic Life Support (BLS) Certification
