Job summary

Corewell Health's Visiting Nurses Association serves the West-Central Michigan home care region, offering flexible schedules including every 4th weekend and a rotating 3 holiday schedule. This autonomous RN role lets you apply top clinical skills and judgment while caring for patients at home, visiting 5–7 patients per shift from Ludington to Gun Lake and surrounding areas.

Qualifications

• Associate's Degree in Nursing required.
• MI RN license upon hire required.
• Driver's license upon hire required.
• BLS certification required (AHA/ARC) within 90 days or as per policy.

Responsibilities

• Communicates with staff, residents and families to develop individualized care plans.
• Administers medications according to guidelines and notifies MDS of changes.
• Collaborates with interdisciplinary team to ensure continuity of care and patient advocacy.
• Educates patients and families; identifies educational needs and provides appropriate referrals.
• Leads and delegates to support staff to ensure safe, high-quality care.
