Job summary

Corewell Health is seeking a dedicated Registered Nurse for our Home Care West-Central Michigan team in Grand Rapids. The role emphasizes autonomous, patient-centered care in the home, with a schedule including 40 hours weekly and every fourth weekend.

Responsibilities include medication administration, care plan development with the interdisciplinary team, and patient education, ensuring high-quality, cost-effective care in patients' homes across the region.

Qualifications

• Must be a licensed RN in Michigan with current BLS.
• Associate's Degree in Nursing required; Bachelor's in Nursing preferred.
• Valid driver's license required for home health travel.
• Certification in Basic Life Support (BLS) must be current.

Responsibilities

• Communicate with staff, residents, and families to develop individualized care plans.
• Administer medications per regulatory guidelines and inform MDS coordinator of changes.
• Coordinate with interdisciplinary team to ensure continuity of care.
• Educate patients and families and provide referrals as needed.
• Lead and supervise nursing staff as appropriate to assure safe, quality care.
