Job summary

BAYADA Home Health Care is seeking an Admissions Registered Nurse (RN) for full-time home health visits in Middlesex, NJ. You’ll conduct initial assessments, establish plans of care, and collaborate with the interdisciplinary team to ensure a smooth transition into home health services.

This role focuses on autonomy in the field, coordinating client needs across Monmouth, Middlesex, Union, and Somerset Counties, with a strong emphasis on comprehensive OASIS Start of Care visits and client

Qualifications

• Current and unrestricted New Jersey RN license.
• Minimum of one year of recent, verifiable clinical nursing experience.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment and care coordination skills.
• Excellent communication, interpersonal, organizational, and time-management skills.

Responsibilities

• Perform comprehensive initial OASIS Start of Care admissions visits.
• Develop individualized plans of care in collaboration with physicians, clients, and caregivers.
• Coordinate nursing, therapy, social work, and other home health services.
• Educate clients and caregivers about diagnoses, medications, and the home health plan of care.
• Document assessments and care coordination in the EMR system and ensure accurate admission documentation.
