Job summary

BAYADA Home Health Care seeks an experienced Admissions Registered Nurse (RN) for full-time work with our UCV office. You will provide home health admissions and OASIS Start of Care visits to adult and geriatric clients across Monmouth, Middlesex, Union, and Somerset Counties, NJ.

As an Admissions RN, you will welcome new clients, complete initial assessments, and establish personalized plans of care in collaboration with an interdisciplinary team, ensuring a smooth transition into BAYADA

Qualifications

• Current and unrestricted New Jersey Registered Nurse (RN) license.
• Minimum of one year of recent, verifiable clinical nursing experience.
• Prior home health experience required.
• OASIS experience required.
• Strong clinical assessment and care coordination skills.

Responsibilities

• Complete comprehensive OASIS Start of Care (SOC) admissions visits for home health clients throughout the assigned territory.
• Perform thorough initial nursing assessments, including medication reconciliation and safety evaluations.
• Develop individualized plans of care in collaboration with physicians, clients, caregivers, and the interdisciplinary team.
• Identify client needs and coordinate nursing, therapy, social work, and other home health services.
• Review and execute physician orders and ensure the plan of care reflects client needs and goals.
• Educate clients and caregivers regarding diagnoses, medications, treatments, safety, and the home health plan of care.
• Communicate clinical findings and changes in condition to physicians and BAYADA care team.
• Ensure timely completion of OASIS assessments and admission documentation.
• Document assessments and care coordination using BAYADA's EMR system.
• Help ensure a smooth transition from referral to admission and ongoing home health services.
