Job summary

Home Services Unlimited in Indianapolis is seeking a compassionate Registered Nurse (RN) Case Manager to provide skilled nursing care in patients' homes across the Indianapolis area. This role offers autonomy, flexible scheduling, and the opportunity to develop lasting patient relationships.

You will develop and evaluate individualized care plans, coordinate with physicians and therapists, supervise LPNs, and ensure documentation and Medicare compliance.

Qualifications

• Active Indiana RN license
• ASN or BSN degree
• Home health experience preferred
• Experience with EHR systems preferred
• Strong clinical assessment, communication, and organizational skills
• Ability to work independently and manage a patient caseload
• Valid driver’s license and reliable transportation

Responsibilities

• Develop, implement, and evaluate individualized care plans
• Provide skilled nursing care and treatments in the home setting
• Educate patients and caregivers on disease management, medications, and care plans
• Coordinate care with physicians, therapists, and other healthcare professionals
• Supervise LPNs and support staff as needed
• Complete accurate and timely electronic documentation
• Ensure compliance with agency policies, Medicare regulations, and clinical standards
• Participate in care conferences, team meetings, and quality improvement initiatives (QAPI)

Responsibilities

As an RN Case Manager, you will:

Job Types: Full-time

Compensation: $50-$110 per visit or Salary for highly experienced candidates.

Work Location: Field-based, providing care in patients' homes throughout the Indianapolis area

Schedule: Flexible scheduling based on patient needs and caseload
