Job summary

BrightSpring Health Services’ Rehab Without Walls Neuro Rehabilitation is seeking a licensed Physical Therapist to deliver functional rehabilitation in home and community settings across Arizona. You will evaluate clients, develop individualized treatment plans, and apply therapy in real-life activities.

This role offers a flexible schedule you help create, paid per hour, mileage reimbursement, and ongoing educational programs with growth opportunities within the company.

Qualifications

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current, unrestricted Physical Therapy license in practicing state.
• Minimum of one year of PT experience preferred.
• Ability to lift up to 100 pounds with safety procedures.

Responsibilities

• Perform evaluations and develop treatment plans.
• Regularly reassess effectiveness; attend staffing meetings and family conferences.
• Perform discharge evaluations and provide home safety recommendations and resources.
• Communicate patient needs and progress to the treatment team, physician, patient, and family.
• Collaborate with other disciplines to coordinate care and improve outcomes.
