Job summary

The Palace Healthcare and Rehabilitation Center is seeking a licensed Physical Therapist to provide comprehensive PT services and develop individualized treatment plans in collaboration with a multidisciplinary team.

You will screen, evaluate, and manage patient care, including discharge planning and supervision of assistants, while ensuring compliance with state and federal regulations. A strong focus on quality outcomes and professional development is essential.

Responsibilities

• Optimize patient functionality by providing, directing, and/or overseeing physical therapy services, including screening, evaluation, and treatment as referred by an approved referral source.
• Communicate and document accurately; ensure billing and charting compliance.
• Maintain patient-centered interactions with patients, interdisciplinary team members, and facility staff.
• Supervise PTAs and Rehab Techs; provide consultation and clinical supervision.
• Participate in utilization review, care conferences, case management, and interdisciplinary activities.
• Develop and enhance physical therapy programs to support quality outcomes.
