The Palace Healthcare and Rehabilitation Center is seeking a motivated Physical Therapist Assistant to deliver quality therapy under the supervision of a licensed physical therapist. You will support individualized treatment plans to improve strength, mobility, and independence, while collaborating with an interdisciplinary team to ensure seamless resident care.

This role emphasizes accurate documentation, active participation in care conferences, and adherence to state guidelines.

Qualifications

• Must have strong clinical problem-solving and decision-making skills.
• Teamwork and collaboration skills essential.
• Excellent written and verbal communication.
• English reading/writing/comprehension required.
• Ability to lift up to 50 pounds.

Responsibilities

• Reports to the Director of Rehabilitation.
• Assist in screening and initial evaluations input as appropriate.
• Implement treatment per the established plan of care.
• Document accurately per guidelines and billing requirements.
• Maintain effective patient-centered interactions with patients and staff.
• Supervise rehab technicians during group and individual sessions.
• Integrate PT services into facility and interdisciplinary treatment plans.
• Participate in care conferences and team meetings as directed.
• Maintain professional relationships with healthcare professionals and colleagues.
• Make referrals to other rehabilitation team members as needed.
• Communicate patient progress and facility issues to the supervising PT.
