Job summary

Countryside Center for Rehabilitation and Nursing is seeking a licensed Physical Therapist to deliver comprehensive therapy services within a collaborative rehab team. You will evaluate patients, create and modify treatment plans, discharge planning, and supervise assistants to ensure high-quality care.

You will engage with an interdisciplinary team in a fast-paced setting, uphold regulatory standards, and pursue ongoing professional development to advance patient outcomes.

Qualifications

• Graduate of an APTA-accredited school of physical therapy.
• Valid state Physical Therapist license.
• Proficiency with Microsoft Office and basic internet navigation.
• Ability to lift up to fifty pounds.
• Strong communication and teamwork skills.
• Ability to work in a fast-paced healthcare setting.

Responsibilities

• Provide PT services and supervise rehab staff per policies.
• Screen, evaluate, and develop treatment plans; discharge planning.
• Maintain professional conduct and comply with regulations.
• Supervise PTAs, Rehab Techs, and students per acts.
• Participate in care conferences and team activities.
• Maintain safety, infection control, and patient rights policies.
• Pursue continuing education and professional development.
• Maintain licensure and stay current with practice developments.
• Understand and apply payment models and guidelines.
• Maintain patient confidentiality and ethical conduct.
• Perform other duties as assigned.
