Job summary

Powdersville Post Acute in Easley, SC is seeking a compassionate Occupational Therapist (OTR) to join our rehabilitation team. You will evaluate residents, develop individualized treatment plans, and provide therapy to help residents achieve independence and improved quality of life.

The ideal candidate holds a Bachelor's in Occupational Therapy with potential for a Master’s/Doctorate. Experience in skilled nursing or long-term care is preferred, plus PCC and Casamba familiarity.

Qualifications

• Bachelor's degree in Occupational Therapy required.
• Master's or Doctorate in Occupational Therapy preferred.
• Experience in skilled nursing, rehabilitation, or long-term care preferred.
• Experience with PCC and Casamba preferred.
• Strong communication, documentation, and organizational skills.

Responsibilities

• Evaluate residents and develop individualized treatment plans based on physician orders and clinical needs.
• Provide skilled occupational therapy services in accordance with established goals.
• Monitor resident progress and modify treatment plans as appropriate.
• Communicate effectively with physicians, nursing staff, residents, families, and interdisciplinary team members.
• Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and timely.
• Participate in care planning meetings, rehabilitation conferences, and discharge planning.
• Educate residents, families, and caregivers on treatment goals and home programs.
• Conduct in-service training and educational programs for facility staff.
• Ensure compliance with all state practice acts, regulatory requirements, and facility policies.
• Promote resident independence, safety, and overall well-being.
